Life Skills Coach

Hope Network is seeking a compassionate Life Skills Coach to join our Community & Social Services team. In this role, you will empower individuals with disabilities or disadvantages to build daily living, social, and employment skills that support greater independence. You will provide one-on-one and group coaching, develop person-centered goals, teach practical skills (budgeting, self-advocacy, time management), and connect participants with community resources.

Responsibilities

• Provide one-on-one and group life skills coaching to adults with disabilities or disadvantages
• Develop and implement person-centered plans focused on daily living, social, and employment skills
• Teach practical skills such as budgeting, self-advocacy, time management, and community navigation
• Document goals, progress notes, and service outcomes accurately and on time
• Collaborate with families, caregivers, and interdisciplinary team members to support client success
• Connect participants with community resources, employment supports, and educational opportunities
• Promote inclusion, dignity, and independence in all interactions and activities
• Support safe transportation and participation in community-based activities as needed
