We are seeking a Family Stability Case Manager to provide intensive care management to families experiencing housing instability, bringing specialized knowledge of early childhood systems, services, and resources to help families build stability during a child's most critical years of development.

This role combines eviction prevention, family support, and early childhood resource navigation to help families achieve both housing stability and positive outcomes for their children.

ARE YOU THE FAMILY STABILITY CASE MANAGER WE ARE LOOKING FOR?

What will you be doing?

As a Family Stability Case Manager, you will partner with families who are at risk of eviction or experiencing housing instability. You will assess family needs, develop individualized stability plans, connect families to critical resources, and provide ongoing support through their journey.

Leverage specialized knowledge of early childhood systems and services, you will help connect families to resources that support healthy child development, school readiness, caregiver well‑being, and long‑term family stability during a child's earliest years.

Key Responsibilities include:
  • Providing intensive case management services to families experiencing housing instability.
  • Conducting holistic assessments focused on housing, income, family support needs, childcare access, and child development.
  • Developing and implementing individualized stability plans.
  • Connecting families to community resources, including housing assistance, childcare, early learning programs, developmental screenings, home visiting services, and parenting support.
  • Building trusting relationships with families through ongoing engagement and follow‑up.
  • Collaborating with housing providers, early childhood organizations, and community partners.
  • Providing warm referrals and advocating for families as they navigate systems of care.
  • Maintaining accurate and timely documentation of services, referrals, and client progress.
  • Participating in case conferencing, team meetings, and community collaborations.
  • Staying current on housing resources, early childhood services, and community support programs.
