Skilled Nurse - LPN

Guardian Angel Home Care is seeking a compassionate and skilledLicensed Practical Nurse (LPN) to join our growing home health team in the Grand Rapids area. In this role, you will provide high-quality, patient-centered care in the comfort of patients' homes while working alongside a supportive interdisciplinary team.

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician
• Monitor patient condition, vital signs, and overall health status
• Administer medications and treatments as ordered and within the LPN scope of practice
• Accurately document all visits and care provided in Homecare Homebase (HCHB)
• Communicate changes in patient condition to the supervising RN and physician
• Educate patients and families on medications, disease management, and the plan of care
• Ensure compliance with all federal, state, and agency regulations
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes

• ActiveMichigan Licensed Practical Nurse (LPN) license in good standing
• Graduate of an accredited practical nursing program
• CurrentBLS/CPR Certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical, communication, and documentation skills
• Ability to work independently in a home health setting

• Home health or hospice experience
• Experience withHomecare Homebase (HCHB) or other electronic medical record systems
• Wound care experience
• Bilingual candidates are encouraged to apply
