Social Worker, Community Health

This is a full-time, on-site Social Worker role based in Granger, IN. The Social Worker will be responsible for providing emotional and practical support to clients and their families, identifying community resources, coordinating care plans, and advocating for client needs. Key tasks include assessing client needs, developing individualized care plans, performing follow-ups, and maintaining accurate and confidential records related to client care.

There will be frequent travel to the South Bend-Elkhart area

• Strong interpersonal, communication, and active listening skills
• Proficient in client assessment, care planning, and case management
• Ability to research and navigate access to community and healthcare resources
• Experience in advocacy, counseling, and crisis intervention
• Attention to detail, record-keeping, and adherence to confidentiality protocols
• Collaboration and teamwork skills with diverse stakeholders
• Bachelor’s or Master’s degree in Social Work or a related field
• Licensed Clinical Social Worker (LCSW) certification or equivalent is highly preferred
• Familiarity with healthcare systems or prior experience in home healthcare is a plus
