The RN II Advanced Primary Care role provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advanced practitioners, the RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits between regular office visits, assisting in panel management of patients, and providing individualized care to improve patient outcomes.
Reports to the respective Advanced Primary Care Lead APP.
Position DetailsNow Hiring: RN– Advanced Primary Care (Virtual Care)
Hybrid after training:
Must be within 1 hour of the Chesterfield Virtual Care Office.
- Build strong patient relationships and monitor vital sign trends.
- Conduct virtual visits between regular office appointments.
- Support chronic disease management, discharge follow-up, and acute care needs.
- Advance guideline‑directed medical therapy (GDMT) and facilitate advance care planning for Medicare and MA patients.
- Collaborate with primary care clinicians in ambulatory settings to coordinate care.
- Perform virtual assessments, patient education, triage, and escalation of care needs.
- Assess medically complex patients and acute exacerbations of chronic conditions.
- Coordinate care with a multidisciplinary team including specialists, social workers, physical therapists, and palliative care providers.
- Document all care provided and maintain patient confidentiality.
- Practice within the RN scope under the supervision of an advanced practitioner.
- Active RN license in applicable state(s).
- Experience in chronic disease management and virtual care preferred.
- Strong communication and collaboration skills.
