• Provide direct patient care, including assistance with activities of daily living (ADLs), vital signs, and medication management
• Develop and implement personalized care plans in collaboration with the healthcare team
• Monitor and report changes in patient condition, medication, and treatments
• Maintain accurate and detailed records of patient care and treatment
• Assist with resident transfers, ambulations, and mobility exercises

• Must be a graduate of an approved CNA training program and certified in the state of Illinois
• Possess excellent communication, interpersonal, and problem‑solving skills
• Ability to work effectively in a fast‑paced environment and prioritize tasks accordingly
• High school diploma or equivalent required; additional education in a health‑related field preferred
• Must be able to lift, push, or pull a minimum of 50 pounds and stand for long periods
