Position DescriptionThe Nurse Practitioner role involves providing high-quality, comprehensive healthcare services to a diverse patient population. The NP will work in collaborative environment with physicians and other medical support staff to ensure the delivery of accessible and compassionate care. This role includes both direct patient care and patient education, as well as supporting our community health initiatives and preventive care.
Essential Duties and Responsibilities- Provide direct clinical care to a diverse patient panel in an outpatient clinic setting, ensuring comprehensive care across a range of health conditions.
- Perform health assessments, including physical exams, diagnostic tests, and treatment plans, while managing acute and chronic health conditions.
- Administer immunizations, prescribe medications, and provide ongoing health education to patients.
- Conduct preventative care services, including health screenings, wellness checks, and chronic disease management.
- Collaborate with physicians, specialists, and other healthcare providers in managing patient care, referring patients as necessary for specialized services.
- Participate in patient care management for individuals of all ages, including pediatric, adult, and geriatric populations.
- Provide culturally sensitive care while addressing social determinants of health and working with patients from underserved communities.
- Work collaboratively with multidisciplinary teams, including medical assistants, nurses, case managers, and behavioral health specialists, to provide holistic patient care.
- Participate in community outreach programs to promote health education, disease prevention, and health awareness.
- Attend all relevant clinical department meetings, pertinent health care facility site meetings, and any additional meetings that may arise, necessitating the provider's participation.
- Document all patient encounters in the Electronic Health Record (EHR) accurately and in a timely manner.
- Ensure compliance with healthcare policies, procedures, and regulations, including those specific to FQHCs, such as the Health Center Program and HIPAA guidelines.
- Maintain continuing education and professional development to stay current with evidence-based practices and clinical guidelines.
- Act as a role model for the healthcare team in terms of professionalism, patient care, and communication.
- Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
- Comply with applicable laws and regulations with respect to collaborative agreements.
- Comply with SIHF Healthcare policies.
- Perform other duties as assigned.
- Extensive knowledge of primary care practices, including management of acute and chronic conditions, health promotion, and disease prevention.
- Strong understanding of patient care management across diverse populations, particularly in underserved communities.
- Ability to work effectively with a wide range of healthcare professionals in a collaborative team-based environment.
- Strong communication skills, with the ability to educate and engage patients in their healthcare decisions.
- Proficiency in the use of electronic health records (EHR) and other healthcare technology.
- Ability to manage a diverse caseload while balancing patient care with administrative responsibilities.
- Cultural competency to work with patients from a variety of racial, ethnic, and socioeconomic backgrounds.
- Ability to educate patients on lifestyle modifications, preventive care, and self-management of chronic conditions.
