ResponsibilitiesDocument visits thoroughly, participate in care coordination, report patient needs and changes, communicate effectively with patients, families, and healthcare team, and adapt to diverse home situations.
QualificationsCurrent unrestricted LPN licensure, valid driver's license and reliable transportation. Preferred: CPR certification, home care experience, strong communication and organizational skills, ability to work independently, and flexible hours.
CompensationTotal annual cash pay ranges from $51,738 to $77,606, including hourly and per visit pay, based on productivity and performance. Benefits include comprehensive packages, recognition programs, 401k, and stock options.
