Summary of the PositionThis position ensures All Care meets all compliance and administrative requirements for the Community Health Assessments (CHAs) and the Community Health Improvement Plans (CHPs) for all four counties within All Care’s service area. This includes collaborative partnerships with local public health authorities, Community Mental Health Programs, Local Planning Committees, Hospitals, other CCOs, and Federally Recognized Tribes. Tracking tools used for the CHA/CHP will be maintained and reported on as required.
Additionally, this position will participate and engage with regional critical access hospitals in the development of Community Health Needs Assessment (CHNA).
As a subject matter expert, this position manages key relationships involving Social Determinants of Health and Equity (SDOH-E), emergency management that impacts health priorities and strategies, participates in equity focused initiatives; community engagement strategies, and may be involved in internal compliance and auditing activities as needed.
- Lead administrative oversight and maintenance of Community Health Assessments (CHA) and Community Health Improvement Plans (CHIP).
- Coordinate CHA/CHP efforts with Community Advisory Councils across Jackson, Josephine/Southern Douglas, and Curry counties.
- Assist in the management of media and press communications; serve as All Care’s primary liaison to local media and community partners.
- Provide advocacy, communication, and reporting to the Board of Governors and organizational leadership.
- Lead and participate in the development and management of community outreach strategies.
- Support project management activities for HRSN programs, Community Information Exchange, the Community Engagement Plan, and the Health Equity Plan.
- Collaborate with emergency officials and internal departments during emergency events.
- Manage and contribute to the development, implementation, and evaluation of CHA/CHIP processes.
- Maintain current knowledge of contract requirements, OARs, and Oregon statutes related to CHA/CHIP, CACs, community engagement, and SDOH/HRSN.
- Ensure compliance with all CHA/CHIP reporting, documentation, and public posting requirements.
- Develop and maintain CHA/CHIP policies and related activities.
- Publish the annual CHP Progress Report.
- Assist with annual OHA CCO deliverables across departments to meet reporting and compliance requirements.
- Ensure accessibility of CHA/CHIP Steering Committee meetings and related activities for individuals with disabilities, limited English proficiency, and diverse cultural backgrounds.
- Engage local governments, Tribal organizations, community partners, and priority populations to assess community health needs.
- Attend community meetings, serve on boards and councils, and cultivate partnerships to support community health initiatives.
- Lead collaborative efforts with regional local public health authorities, Community Mental Health Programs, Local Planning Committees, Hospitals, other CCOs, and Federally Recognized Tribes to align community health improvement strategies.
- Engage CCO leadership, community partners, and CACs to ensure alignment with the CHIP, Quality Improvement Plan, Health Equity Plan, CHNA, and Community Engagement Plan.
- Assist with management of CHP implementation, budget tracking, and alignment with All Care projects and investments.
- Assist with All Care’s community grants spending programs.
- Participate in the external communications strategy, including media relations, press releases, and public messaging.
- Provide regular updates, reports, and presentations to OHA, the CCO Board, and All Care leadership.
