Physical Therapist - Home Health | Competitive Salary $105K-$115K

We are seeking a compassionate and skilled Physical Therapist to join our growing home health team. This rewarding position offers the opportunity to provide personalized, one-on-one patient care in the comfort of patients' homes, helping individuals regain mobility, independence, and improve their overall quality of life through evidence-based therapeutic interventions.

• Conduct comprehensive physical therapy evaluations including physical assessments, functional mobility testing, and detailed medical history reviews
• Design and implement customized treatment plans tailored to each patient's specific needs, goals, and home environment
• Deliver therapeutic exercises, manual therapy techniques, and rehabilitation training programs to optimize patient outcomes
• Provide patient and family education on injury prevention, home safety modifications, and wellness strategies
• Monitor, measure, and document patient progress toward established rehabilitation goals and functional milestones
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate comprehensive patient care
• Maintain accurate, timely clinical documentation and patient records in compliance with state and federal healthcare regulations
• Participate in interdisciplinary team meetings and care plan conferences

• Master's Degree in Physical Therapy from an accredited program (DPT preferred)
• Minimum 1 year of professional experience as a Physical Therapist in home health care settings
• Current, active Physical Therapy license in the state where services will be provided
• Valid CPR/BLS certification (must maintain throughout employment)
• Valid driver's license with reliable transportation and ability to travel to patient homes and multiple business locations
• Strong clinical reasoning and critical thinking skills
• Excellent interpersonal communication and patient education abilities
• Proficiency with electronic medical records (EMR) and documentation systems
• Commitment to accepted professional standards, ethical practices, and continuing education
