Occupational Therapist – Home Health

Work in the comfort of patients’ homes to help them regain independence after illness, injury, or hospitalization. You will deliver individualized therapy, collaborate with the care team, and support patients, families, and caregivers in creating a safe and functional home environment.

• Evaluate patient functional status and independence with daily activities
• Assess home environments for safety risks and barriers
• Develop individualized treatment plans and therapy goals
• Teach adaptive techniques for daily living skills
• Recommend and train patients on assistive equipment
• Educate caregivers and family members on safe mobility and self‑care
• Supervise Certified Occupational Therapy Assistants when appropriate
• Collaborate with interdisciplinary home health team members

• Graduate of an occupational‑therapy program accredited by ACOTE
• Eligible for or certified by NBCOT
• Licensed occupational therapist in the State of Oregon
• Strong communication and organizational skills
• Current CPR certification
• Valid driver’s license and reliable transportation
• Two years of clinical experience is preferred

• One‑on‑one patient visits in a home‑based setting
• Flexible scheduling and routing in a field‑based role
• A wide variety of patient diagnoses and functional challenges
