As a Hospice CNA, you are often the person patients and families see the most. You will:

• Provide personal, hands‑on care in patients’ homes
• Help patients maintain dignity, comfort, and quality of life
• Support families during emotional and meaningful moments
• Be the eyes and ears of the care team, identifying changes in condition
• Help patients remain safely and comfortably at home

• Is compassionate, patient, and dependable
• Takes pride in providing excellent personal care
• Builds trust quickly with patients and families
• Notices small changes and communicates them clearly
• Is comfortable working independently in patients’ homes
• Understands hospice is about both care and connection

• Providing personal care (bathing, dressing, toileting, hygiene)
• Assisting with mobility and range of motion exercises
• Taking and recording vital signs
• Maintaining a clean and safe patient environment
• Reporting changes in condition to the nurse or case manager
• Supporting patients and families with compassion and respect
