Physical Therapist
Grants Pass, Oregon
Contract | Days | 13-Week Assignment
Key Responsibilities:
- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Deliver direct patient care focused on improving mobility, strength, and functional independence
- Document patient progress, treatment sessions, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient goals and discharge planning
- Educate patients and family members on home exercise programs and safe functional techniques
Required Qualifications:
- Graduate of an accredited Physical Therapy program
- Current and valid Physical Therapist licensure in the state of Oregon (or eligibility to obtain)
- Current CPR/BLS certification
- Experience working in a skilled nursing facility (SNF) setting
Preferred Experience / Skills:
- Prior travel or contract therapy experience
- Familiarity with MDS documentation and Medicare Part A/B billing in a SNF environment
