We are seeking a Hospice Clinical Intake Coordinator who understands how we respond in that first moment truly matters.
This role plays a critical part in ensuring patients and families feel supported, informed, and cared for from the very beginning.

You are the gateway to hospice services     You work closely with hospitals, providers, and community partners     You coordinate admissions from referral to start of care     You ensure equipment and services are in place before arrival     You collaborate with clinical and operational teams     You directly impact patient experience and timely access to care      This role requires heart, organization, and accountability.
What You’ll Be Doing (High Level)
• Maintain patient medical records and documentation    
• Verify benefits and obtain insurance authorizations    
• Answer and route incoming phone calls    
• Enter and update patient information accurately    
• Receiving and reviewing hospice referrals    
• Speaking with patients and families to gather information and explain services    
• Coordinating with physicians and referral partners    
• Confirming eligibility documentation and required orders    
• Scheduling admissions and coordinating clinician assignments    
• Supporting smooth transitions from hospital or facility to home    
• Identifying and resolving barriers prior to admission    
• Maintaining accurate and timely documentation      You serve as the bridge between referral and compassionate care.
