Guaranteed salary with earning potential of $300k+

• Provide direct patient care through office visits, telehealth, and same-day appointments
• Perform comprehensive assessments, order/interpret labs and imaging, and diagnose acute and chronic conditions
• Create personalized treatment plans including lifestyle medicine, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document patient encounters in Next Gen EHR
• Participate in team meetings, quality improvement initiatives, and ongoing training
• Contribute to a positive, supportive, team-oriented culture

• Active Oregon license as a Nurse Practitioner (FNP, AGNP, AGNP-PC), PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or related specialty
• Minimum 2 years of clinical experience preferred
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Ability to thrive in a fast-paced, collaborative clinical environment
• Proficiency with electronic health records (Next Gen preferred)
