Associate Wellness Director LPN

Overview

Cascade Living offers independent, assisted and memory care services across Grants Pass, Roseburg, Medford and Ashland. We seek an Associate Wellness Director LPN to provide medication management and skilled nursing for residents.

• Maintain highest quality and consistency in resident care.
• Implement and coordinate residents’ personal care plans.
• Assist in training and monitoring medication administration.
• Demonstrate strong organizational and time‑management skills.
• Monitor caregiver performance to ensure quality care.
• Educate residents and staff on health maintenance and disease prevention.
• Promote Cascade Living’s mission, values and beliefs.

• Current state license as a Licensed Practical Nurse (LPN).
• Compassionate, professional representation of the community.
• Effective communicator with staff, residents and families.
• Ownership mentality, able to stay focused amid interruptions.
• Strong judgment, problem‑solving and decision‑making skills.
• Ability to work flexible hours including evenings and weekends.

$80,000 – $90,000 annually, depending on experience.

• 40 hours weekly
• 5 days per week
• Rotating on‑call and weekend availability required
