Registered Nurse (RN) – Case Manager

Bristol Hospice – Rogue Valley, OR / Grants Pass, OR

Full Time, Hourly
Salary: $48.00 – $52.00 per hour
Shift: Any

Four 10‑hour shifts:
Wednesday – Saturday, 8:30am – 6:pm.

Jackson, Josephine, and Douglas Counties.

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one‑on‑one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician‑ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition to ensure timely and effective responses.
• Keep detailed and up‑to‑date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to additional supportive duties as part of a compassionate, mission‑driven team.

• A valid Registered Nurse license in good standing in the state where you will work.
• A valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.
• Flexible, team‑oriented attitude and willingness to support additional duties as needed.
