Position: Transitions of Care RN — Flexible Schedule & Care Coordination
All Care Management Services in Grants Pass, Oregon, is looking for a Registered Nurse (RN) to join the Population Health Department. The RN will coordinate healthcare for Medicaid and Medicare members, manage care transitions, and provide clinical supervision. This role requires an accredited nursing degree and a valid Oregon RN license, along with at least two years of experience in case management.
