Home Health LPN- $10K Sign-On Bonus

Health Care Support is seeking a Licensed Practical Nurse (LPN) in Grants Pass, OR and surrounding areas with a 4.5-star rated healthcare provider. As a Home Health LPN, you will have the opportunity to work at the top of your license while providing one-on-one care to patients in their homes.

• Provide follow‑up care to patients after discharge from hospital, rehabilitation, skilled nursing, or long‑term care facilities
• Complete daily paperwork to maintain accurate and complete medical records

• Monday‑Friday 8am‑5pm

• Up to $39.85/hour
• $10K Sign‑On Bonus

• Active Oregon Licensed Practical/Vocational Nurse
• Valid CPR certification
• At least 1 year of acute care experience (Home Health, Med‑Surg, ED, SNF, etc.)
• Valid driver’s license and insured vehicle
