What You'll Be Doing- Reviewing clinician OASIS accuracy and provide feedback to clinicians
- Supporting clinicians with complex patient situations
- Monitoring quality measures, regulatory compliance, and patient outcomes
- Assisting with case conferences, care planning, and clinical decision-making
- Collaborating with physicians, intake, scheduling, and operational leaders
- Helping drive initiatives that improve patient satisfaction, quality metrics, and hospitalization rates

- Active Oregon Registered Nurse (RN) license
- Current BLS certification (AHA)
- Previous Home Health experience required
- Experience with Homecare Homebase (HCHB) required
- Strong OASIS knowledge and experience required
- Previous clinical leadership or supervisory experience required
- Excellent communication, coaching, and organizational skills
- Valid driver's license and insured vehicle
- Strong clinical judgment and a passion for developing others
