• Perform comprehensive assessments on all assigned patients, documented in the electronic medical record.
• Communicate changes in patient condition promptly.
• Utilize appropriate nursing diagnoses and data to derive realistic outcomes and document measurable goals on the plan of care; modify the plan as indicated.
• Identify and prioritize patient needs or nursing diagnoses; document accordingly.
• Collaborate with patients, families, and other health‑care team members when planning care; assess discharge plans and make appropriate referrals.
• Evaluate and document patient response to nursing interventions and adjust the plan of care as needed.
• Ensure completed discharge instructions, medication reconciliation, and provide information about the patient portal and other resources.

• Support patient, family, spiritual, and cultural needs; assess learning level and readiness; document education records.
• Use teach‑back methodology for medication side effects and new medication education.
• Distribute educational materials such as Krames for clinical conditions.
• Use culture vision as a resource in care planning and delivery.
• Treat patients, families, and staff with respect and dignity; acknowledge the contributions of significant others.
• Adhere to ethical standards of practice; call for an ethics consult when appropriate.

• Implement nursing interventions consistent with evidence‑based care.
• Maintain and advance the professional image of nursing through use of best available evidence and resources such as Ovid, Up To Date , and Culture Vision.
• Actively participate in clinical improvement projects and unit initiatives.
• Ensure safe medication administration using bar coding, promote health and safety, and complete accurate bedside hand‑off reports.
• Hourly rounds with patient‑care technicians; protect patients by maintaining a safe environment.
• Meet unit‑based and organizational quality metrics and prevent complications such as pressure ulcers, falls, CAUTIs, and central line infections.
• Participate as a unit safety facilitator and in quality initiative activities.
• Attend unit huddles and engage in continuous improvement.

• Oversee and direct patient‑care or support techs; maintain accountability for assigned patients.
• Demonstrate effective interpersonal skills in leading and directing others; address conflict directly.
• Serve as charge nurse/preceptor when assigned; coordinate patient care for the unit.
• Takes clinical lead when patient condition changes and consults primary care physician for treatment or transfer decisions.
• Act as a resource and role model for new staff, students, and personnel.
• Assist with precepting, unit orientation, and mentoring.
• Provide regular feedback to colleagues through peer evaluation and case reviews.
• Collaborate with all hospital services to accomplish daily work and maintain excellence.
• Identify barriers to effective practice and communicate with leadership and operations.
• Ensure appropriate care orders are documented for all admissions.
• Adhere to charging supplies and initiate unit/organizational initiatives.
• Commit to decreasing utilization and overtime hours.
