Youth Services Support Program Coordinator
Great Falls, MT (School-Based / Community) | Full-Time | Non-Exempt
Reports to:
School & Community Programs Director
Important note: This position is funded through a federal grant that ends June 2028
. It is a permanent-contingent role (permanent status, contingent on continued grant funding). Employment may end if funding is reduced, discontinued, or expires.

The Role
The Youth Services Support Program Coordinator helps run the Youth & Family TREE Program
, a federally funded initiative supporting adolescents, transitional-aged youth, and families impacted by substance use disorders (SUD) and co-occurring mental health conditions
.
You’ll keep the program moving day-to-day: scheduling, coordination, communication, event support, partner follow-up, and—most importantly—
accurate data tracking and reporting required by the grant. You’ll work closely with the Program Director, internal care teams, and community partners to create smooth “warm handoffs” and strong engagement.
What You’ll Do
- Coordinate daily program operations: scheduling, documentation support, and team communications
- Serve as a liaison between participants and internal/external service providers to support access and warm handoffs
- Collect, enter, and maintain accurate participant/program data to meet grant reporting and compliance requirements
- Support referrals to other Alluvion Health services when appropriate (medical, dental, outreach/enrollment, prescription assistance)
- Help plan, organize, and attend youth-focused events, coalition meetings, outreach/tabling, and community engagement activities
- Assist with outreach materials, presentations, informational handouts, and public messaging (including social media support as assigned)
- Support program quality improvement, audits, and grant-required fidelity standards
- Maintain HIPAA compliance and confidentiality in all interactions
- Perform other duties and special projects as assigned
What We’re Looking For
Required:
- 3–5 years of related experience
- Strong organizational skills and attention to detail (especially with data accuracy)
- Strong communication skills and customer service mindset
- Ability to collaborate with diverse teams and community partners
- Proficiency with common office technology (Outlook, Word, Excel, internet tools); ability to learn an EMR/EHR
- Valid Montana driver’s license
- Ability to pass a criminal background check
Preferred:
- Associate’s or Bachelor’s degree in Social Work (or related field)
- Experience in a healthcare setting and/or FQHC
- Certifications or experience such as Behavioral Health Aide, Mental Health Aide, Medical Assistant, Chronic Care professional, Certified Application Counselor, and/or SHIP
Work Environment & Physical Requirements
- Indoor, climate-controlled environment ~95% of the time
- Occasional lifting/moving up to 50 lbs (files, supplies, equipment)
Regular computer work and document review - Travel to other clinic sites and community locations as needed
- OSHA Exposure Category #2 (no routine exposure, but potential exposure may be required as a condition of employment)
