Licensed Practical Nurse (LPN) Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in residents' conditions to ensure prompt intervention and treatment

• Collaborate with other healthcare professionals to ensure continuity of care

• Maintain accurate and detailed records of resident care, including medication administration and treatment plans

• Participate in quality improvement initiatives to enhance patient care and outcomes

Licensed Practical Nurse (LPN) Requirements:

• Current LPN licensure in the state of New York

• Prior experience as an LPN; prior experience in long-term care or sub-acute settings a plus
