Human Touch Home Health is seeking a licensed Physical Therapist (PT) for a PRN (as-needed) home health position serving patients in ZIP codes 20772, 20782, 20783, and 20785 in Prince George’s County, Maryland.

The Physical Therapist will provide skilled therapy services in the home setting, supporting patients in improving mobility, strength, balance, and overall function. This position requires independent clinical judgment and coordination with the care team.

• Perform in-home PT evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide therapeutic interventions per physician orders
• Complete documentation accurately and timely
• Educate patients and caregivers on home exercise programs
• Collaborate with physicians and interdisciplinary team members
• Supervise PTAs as applicable

• Active Maryland Physical Therapist license
• Minimum 1 year of experience (home health preferred)
• OASIS experience preferred
• Strong clinical and documentation skills
• EMR experience preferred
• Valid driver’s license and reliable transportation
