- Current Physical Therapist Assistant (PTA) license in the State of Colorado or eligibility through compact licensure
- Graduate of an accredited Physical Therapist Assistant program
- Strong communication and documentation skills
- Passion for providing compassionate, resident-centered care
- Ability to work independently and as part of an interdisciplinary team
- Provide physical therapy treatments under the direction of a licensed Physical Therapist in a skilled nursing/long-term care setting
- Accurately document resident progress and treatment plans in accordance with regulatory requirements
- Collaborate with physicians, therapists, nursing staff, and interdisciplinary team members to achieve optimal resident outcomes
- Follow all applicable state and federal regulations governing therapy services
- Adhere to facility policies, procedures, and clinical standards
- Promote resident safety, independence, and quality of life through individualized care plans
- Travel to assigned facilities throughout Southern Colorado as needed
