Physical Therapist, PT Home Health

Overview

A home health care provider is seeking a full-time Physical Therapist to deliver patient-focused rehabilitation services in the comfort of patients' homes. This role is based in Loveland, CO, serving the local community.

• Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately
• Create and implement tailored treatment plans that address individual rehabilitation goals and patient needs
• Conduct therapeutic exercises and training programs to improve mobility and enhance overall functionality
• Educate patients and their families about injury prevention, wellness strategies, and the recovery journey
• Monitor and document patients' progress, adjusting treatment plans as necessary to achieve rehabilitation goals
• Collaborate with a multidisciplinary healthcare team to ensure holistic patient care
• Maintain meticulous patient records and documentation, complying with regulatory standards

• Minimum nine months experience as a physical therapist
• Community/home health experience preferred
• Master's degree in physical therapy from a CAPTE accredited physical therapist education program
• Currently licensed to practice as a physical therapist within the state of agency operation, compliant with accepted professional standards and practices
• Valid CPR certification, maintained while employed in a clinical role
• Must be a licensed driver able to travel to all business locations
