Bilingual Transitions-of-Care APP — Virtual Care Lead

Devoted Health, Inc. is seeking a full-time Advanced Practice Provider for their Medical Transitions of Care program. This role focuses on providing virtual care after patient discharge to prevent readmissions through comprehensive, wrap-around care.

The ideal candidate will have 5+ years of experience managing geriatric populations and strong competencies in medication reconciliation. A combination of RN and APRN licenses or PA license in good standing is required.

Base salary ranges from $120,000 to $150,000 annually, with additional performance-based bonuses available.
