Position Summary

The Director of Care Coordination provides strategic leadership and operational oversight of Northeast Health Partners’ (NHP) care coordination program. This position is responsible for ensuring the delivery of high-quality, person-centered, and integrated care coordination services for members with complex physical health, behavioral health, substance use disorder, and social needs. The Director leads the development, implementation, and continuous improvement of care coordination strategies to ensure services meet member’s needs, achieve measurable outcomes, and comply with applicable state, federal, and contractual requirements.

The position provides leadership and direction to care coordination staff, including oversight of staffing, supervision, training, performance management, workload distribution, and program operations. The Director is responsible for ensuring timely and effective identification, outreach, assessment, care planning, care transitions, and coordination of services for members, with particular attention to individuals with complex or high-risk needs. The position promotes a whole-person approach that integrates physical health, behavioral health, substance use disorder services, and health-related social needs.

The Director works collaboratively with NHP leadership and departments, providers, hospitals, community organizations, county agencies, schools, state partners, and other stakeholders to strengthen care integration, address barriers to services, and improve member outcomes. The Director monitors program performance, utilization, quality measures, member outcomes, and contractual deliverables and uses data to identify opportunities for improvement. This position ensures appropriate documentation, reporting, audit readiness, and adherence to NHP policies and regulatory requirements.

The Director serves as a key organizational leader responsible for advancing health equity, strengthening member engagement, supporting effective transitions of care, and ensuring NHP's care coordination program delivers meaningful and measurable value to the members and communities it serves.

DUTIES PERFORMED
  • Oversee the design, implementation and operational management of all Care Coordination requirements as outlined in ACC 3.0 Contract.
  • Facilitate effective communication, collaboration, and coordination within and between NHP and partner PCMPs, CMAs, BHASOs, and D-SNPs with the goal of ensuring comprehensive, effective and efficient coordination of care for jointly managed Members.
  • Build and nurture trusted and cooperative working relationships with all partner PCMPs, CMAs, BHASOs, and D-SNPs.
  • Manage Care Coordination processes and staff, providing oversight, monitoring, and training as required.
  • Oversee effective delivery of Care Coordination for Members by Network Providers, delegated and Subcontracted organizations, and others within the Health Neighborhood.
  • Create proactive processes and policies to prevent the exacerbation of Member conditions and the escalation of Member situations.
  • As a member of the Executive Management team, work cooperatively with others to develop and support the vision, mission and values of the agency. Ensure others foster agency vision, mission and values through demonstrated actions and communications within and outside the agency.
  • Provide leadership, motivation and direction to the community partners and ASO, particularly for the programs and initiatives assigned, and to the agency as a whole.
  • Accountable for the development, implementation, and functions of assigned program(s) and is responsible for ongoing administration. May be responsible for the development of agency wide initiatives.
  • Responsible for policy and procedure development and participates in overall agency policy and procedure development and implementation.
