Position Summary

Banner North Colorado Medical Center in Greeley, Colorado is hiring Licensed Practical Nurses (LPNs) for part‑time night shifts in the Oncology and Telemetry unit. The unit has 44 beds with an average daily census of 38 patients. LPNs will provide nursing care under the direction of a registered nurse and/or licensed physician.

Core Functions
  • Observe, compile, and report the patient’s physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Review available information from the health‑care team.
  • Contribute to the plan of care, including discharge planning, using assessment data and input from patients, families, and the health team. Consider the patient’s cultural, psychosocial, spiritual, age‑specific, and educational needs. Plan and update care in collaboration with multidisciplinary team members.
  • Implement delegated interventions identified in the care plan and medical orders within scope of practice. Provide care based on physician orders and nursing plan, complying with policies, procedures, and regulatory requirements. May delegate and supervise non‑licensed personnel, within state scope of practice.
  • Report patient condition, signs and symptoms indicating changes, and progress toward goals to the registered nurse and/or physician. Observe and report response and effectiveness of patient teaching.
  • Document observations, implementation, and data in the patient record. Include objective/subjective data, nursing interventions, and patient responses. Record physician orders accurately and timely.
  • Plan, organize, develop, and coordinate documentation and patient assessment data as required by federal regulation. Update documentation per patient condition changes and federal/state guidelines. Coordinate with interdisciplinary team regarding functional/care level transitions. Perform reimbursement documentation audits and present findings to management.
  • Coordinate admission processes, including new resident evaluations. Communicate verbal and written information to residents, families, referral sources, insurance companies, and clinical staff. Facilitate problem solving with all parties.
  • Work under supervision, prioritizing data from multiple sources to provide quality care and support to patients and families. Operate in a fast‑paced, sometimes stressful environment with a strong customer‑service focus. Interact with staff at all levels throughout the facility.
  • Minimum Qualifications
    • Current, valid LPN or LVN license in the state of practice (or temporary or compact license).
    • BLS certification required.
    • Additional certification or continuing education may be required based on area of practice.
    • Completion of a state‑approved IV therapy program may be required for certain facilities or departments.
    • For Banner Registry and Travel positions: minimum one year experience in an acute‑care hospital, with acute‑care experience within the past 12 months in the specialty area.
    • Employees for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at hire and maintain it throughout employment.
    Preferred Qualifications
    • Experience in the designated clinical area.
    • Additional related education and/or experience.
    Estimated Pay Range

    $25.54 – $38.30 per hour, based on experience, education, certifications, and geographic location.
