POSITION SUMMARY

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and professional nursing practice standards using the framework for professional nursing practice and development. It is accountable for the quality of nursing services delivered by self or others under their direction and utilizes specialized knowledge, judgment, and skills necessary to assess data and plan, provide, and evaluate care appropriate to the physical and developmental age of assigned patients.

FUNCTIONS
  • Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs; review patient history and physical with patient/family ensuring completion within appropriate timeframe; review information from other health care team members; review diagnostics and laboratory data and report abnormal results to physicians and appropriate caregivers; complete assessment and reassessments as per policy.
  • Formulate a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input; initiate a plan based on patient-specific needs, assessment data, and the medical plan of care; set realistic, measurable goals with patient/family; consider physical, cultural, psychosocial, spiritual, age-specific and educational needs; collaborate with the multidisciplinary team; review and revise the plan to reflect changing patient needs.
  • Implement the plan of care through direct patient care, coordination, delegation and supervision of the health care team; provide care based on physician orders and the nursing plan of care, complying with policies, procedures, standards of care and regulatory requirements; delegate appropriately and provide nursing supervision of other licensed nurses and personnel; promote continuity of care by communicating patient status accurately to other caregivers.
  • Evaluate the patient and family’s response to care and teaching, and revise the plan as needed; evaluate patient progress toward goals and expected outcomes in collaboration with the health care team; evaluate effectiveness of patient teaching.
  • Document assessment, planning, implementation and evaluation in the patient record; ensure documentation is legible, timely and conforms to policy; record objective/subjective data, nursing interventions and patient response; note physician orders accurately and timely.
  • Provide care based on best evidence; may participate in research activities within clinical practice; participate in unit or facility shared leadership; engage in education, role development and orientation of facility personnel, patients, students, families and visitors; promote/support growth of others through precepting and mentoring when appropriate.
  • Contribute to society through activities that lead to excellent patient outcomes via timely, effective, efficient, equitable, and safe care; actively participate in improvement of national nursing quality indicators and outcomes; participate in professional organizations as appropriate.
  • In some roles, supervise staff and workflow of the department.
  • Interact with all levels of staff in various departments, physicians, patients, families and external contacts, including other health care institutions, community providers and agencies concerning patient health; work with clergy to provide spiritual support; synthesize and prioritize data from multiple sources to support the patient and family’s response to changes in health status.
MINIMUM QUALIFICATIONS
  • Current, valid RN license in state of practice, temporary/interim RN license, or compact RN licensure for current state of practice.
  • BLS certification is required.
  • Additional certification or continuing education may be required based on area of practice.
  • Experience in the clinical area for which applying is desired.
  • Maintain clinical performance competencies appropriate to the area of practice, demonstrated through annual validations.
  • For Banner Registry and Travel acute care positions: minimum one year experience in an acute care hospital setting; must have worked in an acute care setting within the past 12 months as a Registered Nurse in the specialty area.
  • For Banner Registry and Travel physician practice positions: minimum one year experience as a Registered Nurse in a physician practice or acute care setting.
  • For Home Health: must be able to operate personal vehicle, possess a valid driver’s license; active auto insurance coverage required.
  • For specific behavioral health positions (e.g., Banner Behavioral Health Hospital, BTMC Behavioral, BUMG, BUMCT, BUMCS, Banner McKee Senior Behavioral Health Inpatient Unit): possess the respective state fingerprint clearance card (Arizona or Colorado) at hire and maintain it;
    Arizona Criminal History Affidavit required upon hire.
