• 45‑minute visit conducted in the patient’s home
• Mandatory medication and medical history review
• Mandatory physical evaluation and optional diagnostic tests (e.g., diabetic eye exam, spirometry)
• Use of an iPad and an intuitive clinical workflow
• No prescribing, ordering labs, or mandate to alter current treatment regimen

• Provide in‑home health evaluations and build trusted relationships with patients
• Connect patients, family, and care team to appropriate community resources
