Registered Nurse

Life Care Centers of America seeks a compassionate Registered Nurse to provide high-quality care to seniors in our rehabilitation and long-term care community. In this patient-first, family-focused environment, you will assess and monitor residents, administer medications and treatments, and coordinate individualized care plans with an interdisciplinary team. RNs support rehabilitation goals, educate residents and families, and ensure regulatory compliance. Responsibilities

• Provide direct nursing care to residents including assessments, medication administration, and treatments
• Develop, implement, and update individualized care plans in coordination with interdisciplinary team
• Monitor patient conditions, document observations, and communicate changes to physicians and families
• Support rehabilitation goals by collaborating with therapy staff to optimize functional outcomes
• Educate residents and families on care plans, medications, and health maintenance
• Ensure compliance with nursing standards, facility policies, and state/federal regulations
• Supervise and support CNAs and other support staff to ensure high-quality, compassionate care
• Maintain accurate, timely clinical documentation in electronic health records
• Participate in care conferences, shift handoffs, and team meetings to coordinate care
• Promote a safe, respectful, patient-first environment aligned with Life Care Centers' mission
