Job Description Summary Provides specialized support for children and families with complex needs by managing a caseload of children enrolled in Medicaid Waivers within the Case Management Agency (CMA). Oversees high acuity cases, facilitates transitions from children to adult waivers, and coordinates emergency and exception processes. Strengthens partnerships with families, community providers, and interdisciplinary professionals to support stability, promote service quality, and ensure cohesive care planning.
Contributes to system improvements through specialized children's services expertise, training, and knowledge sharing.
Primary office location will be in the Greeley office. Work may involve time away from a standard office environment, which may consist of driving to and from home visits. Work may occur in various community settings that are potentially hazardous and may require additional precautions, such as personal protective equipment and/or requesting assistance from emergency medical personnel or law enforcement. Reliable, predictable attendance within department business hours of 8 a.m. to 5 p.m. Monday through Friday.
After hours and on-call work situations may occur, on occasion, to provide support to members and families that need services outside of normal business hours. This position has the opportunity for a hybrid schedule, for employees who have completed initial training and meet criteria outlined in division policy.

Job Description
** Complex Case Management - 40%
*** Maintain a caseload of high acuity and transitioning children as assigned, including Children's Habilitation Residential Program (CHRP) and out-of-state placement cases.
* Convene individualized support team meetings as needed to address case specific issues.
* Provide mediation between members, families, and providers to resolve conflicts and support long-term and person-centered support planning.
* Develop appropriate residential transitions for children/youth in out-of-home placements.
* Coordinate RFP process for placement settings.
* Support members and families enrolled in a children’s waiver during out of county and/or out of state placements.
* Cultivate and sustain effective partnerships with key internal and external stakeholders (e.g. Child Welfare, Family Resource Division, Regional Accountable Entities (RAE)
-Creative Solutions Team and provider-based wrap facilitators) to support integrated service planning and high-quality outcomes.
* Collaborate closely with Weld County School Districts (based on the child's resident school location) to ensure smooth, well-coordinated educational transitions.
* Work with members, families, and community agencies to coordinate and facilitate services.
* Conduct out-of-county travel, when necessary, to support coordinated case management for CMA members/children that are also involved with other DHS programs.
** Waiver Transition Coordination - 15%
*** Coordinate all children’s transitions to adult waivers, including assessments, planning meetings, documentation, and implementation of transition plans.
* Facilitate and manage Developmental Disabilities (DD) Medicaid Waiver enrollment requests.
* Facilitate and oversee all emergency requests and exception processes for children transitioning to the DD waiver.
* Provide information as requested about all potential services and the enrollment process, guiding, and prompting through each step as needed.
