Description

Responsibilities include managing the planning, implementation, and evaluation of the assigned Dementia Program workplan, project management and staff management. The Program Director will coordinate leadership activities. Working as a member of the program team, this role will use advanced analytical, research, project management and relationship management skills to deliver on priority work plan activities including program assessment, planning, implementation and evaluation.

Daily attendance is an essential function of this position. Frequent travel is required to meet the needs of the position.

Principle Duties and Responsibilities
  • Manage the planning, implementation, and evaluation of assigned grant funded program workplan activities. Experience and competency in grant applications and leading grant teams.
  • Assist on-site team with oversight of implementation of project activities, internal and external coordination, development of materials, lead meetings, technical assistance, evaluation, staff training.
  • Coordination of and budget management for memory care activities, programming supplies, and departmental resources.
  • Lead initiatives focused on improving health outcomes and quality of life for Elders, with emphasis on dementia awareness, prevention and caregiver support within Tribal communities.
  • Coordinate partnerships with Tribal Elder programs, health departments, and community-based organizations to enhance culturally grounded dementia education and services.
  • Oversee collection and analysis of data related to elder health, cognitive decline, and social determinants of health to inform program planning and evaluation.
  • Contribute to ongoing strategic planning, budgeting and program support to ensure the successful development and implementation of projects and programs.
  • Gather, analyze and prepare clinical and public health policy, systems and environmental change recommendations.
  • Develop and implement accurate reporting mechanisms that measure the efficacy of previous and current programs, strategies and initiatives and inform ongoing improvements.
  • Support communications through conference calls, meetings, written briefs, publications, events, newsletters and website features.
  • Work with program staff to plan program events, meetings, trainings, conferences, conference calls and webinars.
  • Assist participating tribes in strategic planning, assessment implementation and evaluation of dementia activities and strategies.
  • Engage Tribal leaders in promoting culturally adapted evidence-based practices to address chronic and acute conditions and to promote good health and wellness.
  • Actively participate in task teams, work groups, and other joint policy and program initiatives.
Additional Duties and Responsibilities
  • Maintain a working knowledge of all grants and programs of the department. Engages in networking activities to remain current in the availability of epidemiological and other health data resources available within the service area.
  • Assure all services are delivered in a culturally sensitive manner.
  • Participate in committees outside and within the organization.
  • Attend all training sessions and meetings as scheduled.
  • Attend conferences and training to stay current on advances in the field and obtain new skills.
  • Participate in research projects as designated.
  • Perform other duties as necessary to fulfill the responsibilities of this position.

This is a salaried full-time position working at our office located in Green Bay, Wisconsin. This is not a remote position. The starting wage for this position is $70,000 - $75,000 annually, depending upon experience, with benefits including:
Annual/Personal Leave and Holiday Pay, Health, Dental, Life & AD&D, STD and LTD Insurance, FSA and 401(k) Retirement Plan. Regular work hours are normally Monday through Friday 40 hours a week. There will be variation in work hours due to special projects, deadlines, and to meet the needs of the families served.
