Location: Northern Wisconsin (Green Bay, Oshkosh, Manitowoc, Sheboygan & Grafton Region)
Position OverviewWe are seeking an experienced Hospice Liaison Registered Nurse to coordinate hospice referrals and guide patients and families through end-of-life care transitions. This role involves collaborating with hospital teams, assessing patients for hospice eligibility, educating providers and families, and facilitating admissions into hospice services.
Assignments will typically cover 1–2 hospitals at a time
, with travel throughout the northern Wisconsin region as needed.
- Active RN License
- Minimum 2 years of nursing experience
- Hospice liaison experience preferred; other hospice experience considered
- First-time travelers welcome
- BLS certification required
- Valid Driver’s License, reliable vehicle, and active auto insurance required
- Epic EMR experience required
- Review and manage hospice referrals from hospitals and skilled nursing facilities
- Perform patient assessments and determine appropriate hospice level of care
- Coordinate hospice admissions in hospital settings
- Educate patients, families, and providers on hospice philosophy and benefits
- Collaborate with physicians and interdisciplinary teams to support end-of-life care planning
- Recommend symptom management strategies for pain, dyspnea, and other comfort-focused needs
- Advanced chronic illnesses
- End-of-life and palliative care populations
- Knowledge of hospice philosophy and eligibility criteria
- Experience with end-of-life symptom management
- Understanding of hospice care levels (Routine, General Inpatient, and Respite)
- Strong communication and provider collaboration skills
- Comfortable discussing goals of care with patients and families
- Epic Hyperspace and Epic Remote Client experience
- Dedicated supervisor and manager support
- Experienced Hospice Liaison team
- Access to Hospice Medical Directors
- Company-issued laptop and mobile phone
- New Employee Orientation (NEO)
- 40 hours of unit-specific orientation with an experienced staff member
- Business casual attire or professional scrubs
- White coat preferred but not required
- Free parking provided
- Average workload: 2–3 patient consultations per day
