Registered Nurse

Position Title: Registered Nurse

Location: Park View Health Center

Status: Full and Part-Time

Hours per Week: 8 to 40

Work Hours: Openings on the following shifts

  • Day Shift: 6:30 AM to 2:30 PM
  • PM Shift: 2:30 PM to 10:30 PM ($3/hour shift differential)
  • Night Shift: 10:30 PM to 6:30 AM ($2/hour shift differential)
  • Working Every Other Weekend ($2/hour weekend differential)
Purpose and Summary

Ensures residents receive the necessary care and services through a multidisciplinary approach to attain or maintain their highest practicable physical, mental, and psychological well‑being. Provides nursing care and assessment to establish interventions within a plan of care in an environment that promotes quality of life, in accordance with Park View Health Center’s policies, federal and state regulations, the Nurse Practice Act, and the nursing assessment process.

Functions
  • Delivers skilled nursing care, administers medications as ordered, transcribes physician orders, and ensures direct care is provided skillfully, safely, completely and accurately.
  • Manages, directs, and assigns tasks to certified nursing assistants, delegating direct nursing care tasks as appropriate and providing guidance-driven leadership for the neighborhood team of CNAs.
  • Completes accurate nursing assessments in a timely manner, identifies resident changes in condition, directly assesses and observes residents with a change in condition, documents observations in the medical record, and follows up with appropriate changes to care plans.
  • Additional Essential Duties
  • Accurately and timely documents resident assessments, pertinent resident focused information, weekly resident progress notes, resident/POAHC/Guardian notification, and notification to the resident’s physician/physician extender within each resident medical record.
  • Critically thinks through resident concerns and forms problem‑solving solutions by identifying interventions on resident short‑term care plans, consulting with interdisciplinary team members, discussing concerns directly with residents and family members, and offering assessment information to RN supervisors.
  • Maintains effective and professional communication with physicians, physician extenders, community healthcare entities, interdisciplinary departments, supervisors, and administration to provide assessment information and request consultation or guidance for resident focused concerns.
  • Understands nursing department chain of command and reports acute incidences or concerns in a timely manner to the RN Shift Supervisor and the RN Neighborhood Supervisor, if applicable.
  • Demonstrates ability to organize and prioritize daily assignments, completing medication passes, resident assessments, documentation, and transcription of physician orders within the scheduled shift.
  • Accurately and thoroughly transcribes newly signed physician orders, physician visits, and consultant visits, documenting the information within the resident’s medical record.
  • Demonstrates flexibility in work hours and assignments, adapting to a changing work environment and frequent interruptions; staff nurses will work every‑other‑weekend and be assigned to a Holiday rotation schedule.
  • Monitors neighborhood supplies used for resident care to ensure appropriate quantities are maintained, including medications, skin or wound care supplies, tube feeding or oral nutritional supplements, foley catheters, and adaptive equipment.
  • Completes consents for medications and safety equipment by delivering education and obtaining signatures from the consenting party.
  • Thoroughly assesses resident vital signs, labs, diagnostic tests, and consultation reports for acute concerns and communicates concerns to primary physicians.
  • Participates in the admission, transfer, and discharge processes for residents.
  • Functions as a resident advocate and responds promptly to requests and concerns.
  • Assists certified nursing assistants with resident direct care as able, including transfer assistance, meal‑time assistance, and obtaining vital signs.
