Job Summary
The Advanced Practice Provider Float at Prevea is an advanced practice nurse (NP) or physician assistant (PA) prepared to deliver health care services to patients, families, and groups emphasizing promotion of health and prevention of disease under direct supervision and responsibility of physicians. This float position works 30 hours per week, primarily in the Green Bay area but may travel to other regions and support multiple locations as needed.
WhatYou Will Do
- Provide assessment, evaluation, and management of patients with minor, acute, chronic, and episodic illnesses and injuries, including follow‑up exams as required.
- Collaborate with patients to review their health background and determine physiological, emotional, and environmental needs, using this information to advise patients and families on future health plans and illness prevention.
- Provide case management services to assist clients and families with arrangements for or referral to needed services.
- Order appropriate diagnostic and laboratory tests, prescribe pharmacological agents, and administer other non‑pharmacological treatment modalities.
- Maintain and review patient records, charts, and other pertinent information, documenting all details in the electronic medical record.
- Conduct telephone calls to patients requiring follow‑up care or to provide test results, and provide health counseling and education to patients and families about their healthcare and follow‑up instructions.
- Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes.
- Work at multiple sites with variable shifts at the supervisor’s direction to cover leaves of absence, paid time off, or department demands.
- Graduate of a master’s level nurse practitioner program OR graduate from an accredited physician assistant program.
- 1–3 years of clinical experience (specific experience to be detailed in full listing).
