Job Summary

The Registered Nurse at Prevea plays a crucial role as an integral member of our clinic team, delivering patient-centered care. Responsible for triaging incoming calls from patients and families, assessing their needs, giving appropriate advice, and facilitating appointments with physicians and advanced provider clinicians. The Registered Nurse position will also provide hands‑on direct patient care as needed and is accountable for the delivery of exceptional and safe care within the scope of practice as defined by the Wisconsin Board of Nursing and Prevea’s policy.

Responsibilities
  • Manages patient/family health concerns through telephone triage and consultation.
  • Actively seeks information to understand patient circumstances, problems, expectations, and needs.
  • Facilitates callers’ access to appropriate level of care by assessing needs and offering clinical options using medical judgement within scope of licensure.
  • Might refer to health care providers, services, and community resources.
  • Collaborates with other healthcare providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.
  • Conducts telephone calls back to patients requiring follow-up care or providing test results.
  • Provides health counseling and education to patients and their families about their healthcare and any follow-up instructions.
  • Provides direct patient care, including (but not limited to) the rooming of patients, obtaining medical information, performing comprehensive initial patient assessment, and maintaining patient charts/records.
  • Assists the Physician or Advanced Practice Provider with procedures, notes their instructions/orders, and makes necessary arrangements to ensure they are carried out.
  • Administers medication as needed and communicates information with patients, physicians, advanced practice providers, and staff to ensure quality and continuity of care.
