Key Responsibilities Case Management & Client Support Deliver case management and supportive services to homeless Veterans; maintain accurate case files
Review Intake Specialist assessments and determine participant service needs
Develop Housing Identification and Stability Plans Develop and monitor Single Coordinated Care Plans Execute participation agreements with participants
Create discharge plans with referral options
Housing Stability & Counseling Provide housing stability counseling, including awareness and referral to rental, rent subsidy, and housing assistance programs
Educate participants on supportive services and client rights, including fair housing, landlord-tenant laws, lease terms, rent/mortgage delinquency, home maintenance, and financial management
Referrals & Service Linkages Make program referrals and establish linkages with appropriate agencies, including:
VA non-compensation benefits
Vocational and rehabilitation counseling
Employment, training, and educational assistance
Healthcare and childcare services
Protective payee and legal services
Daily living, emergency, and direct financial assistance
Coordinate transportation services, including pick-up/drop-off and bus tickets
Financial Counseling & Assistance Provide financial counseling and assist participants in developing personal budgets
Request temporary financial assistance after Area Manager/Director approval
Documentation & Compliance Maintain accurate, timely records of client assessments, contacts, progress, and HMIS entries
Other duties as requested or assigned
Key Performance Indicators
90% of participants have updated Housing and Coordinated Care Plans within 30 days of intake
95% of case files and participant documentation are complete and accurate in HMIS
85% of participants meet housing stability goals within the planned timeframe
90% of participants are connected to at least one external service within 30 days of intake
95% of transportation and service coordination requests are completed on schedule
85% of participants develop and follow a personal budget plan
100% of client records updated within 48 hours of service delivery
Zero compliance issues identified in internal or external auditsA bachelor’s degree in social services is required
Minimum two years of experience in case management, life skills as well as conducting assessment and counseling services
Associate’s degree with 4-5 years of experience in case management, life skills as well as conducting assessment and counseling services is acceptable
Experience providing services to veterans is strongly preferred
Experience delivering services to SUD, COD, or homeless populations is also preferred
Valid Wisconsin Driver’s License in good standing and current/valid auto insurance
