Unity Hospice in Green Bay, WI seeks a part-time Social Worker to provide psychosocial support and bereavement services to patients and families at the end of life.
You will perform assessments, create care plans, coordinate resources, and collaborate with an interdisciplinary team, while ensuring compassionate, ethical care and adherence to policies.
Schedule:
24 hours/week, 3 days, with every 4th weekend rotation.
