Summary
Provides basic nursing functions and services to individuals and families under moderate supervision in a clinic, school or community setting. Applies a variety of technical, interpersonal, analytical, and organizational skills to problems of health. Conducts communicable and preventative disease case‑finding investigations to prevent and control diseases and promote and maintain health of populations.
Responsibilities- Performs nursing assessment using CCSG service guidelines.
- Develops and implements a plan of care based on identified health risks.
- Provides basic nursing care consistent with CCSG and nursing scope and standards of practice.
- Documents services according to CCSG and administrative reference standards.
- Maintains confidentiality and privacy.
- Participates in specialized or program‑specific clinics.
- Develops and conducts educational counseling and teaching activities for individual clients and families.
- May serve as a coordinator of a specific program.
- Assesses health status of individuals, groups, and populations through data analysis.
- Identifies at‑risk populations and participates in policy development and continuous review of plans.
- Advocates for at‑risk or high‑risk population groups.
- Provides or assists in providing nursing services to at‑risk or high‑risk populations or those with limited access to care.
- Assists with problem‑solving skills and mentors new staff in agency policies and procedures.
- One year of Registered Nurse (RN) experience.
- Educational equivalency: BSN may substitute for the one year of required experience.
- Must have RN license in Kentucky or a compact state.
- Maintain current RN licensure in Kentucky or a compact state.
- Complete required annual updates.
Must be able to submit to drug screening and background check per agency policies. Probationary period: initial and promotional probationary period of 6 months.
