Gentiva Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to provide hands-on hospice care in patients' homes, assisted living, or facilities. You will report to the Administrator and work with an interdisciplinary team to carry out individualized care plans.
The role requires an LPN license, CPR, and 2–5 years of relevant experience, with 3–5 years preferred. Travel and driver’s license with insurance are required as part of field-based care.
