How You'll Make an Impact as a Speech-Language Pathologist

As a crucial member of our home‑based care team, you will:

* Champion Elara Caring's mission and philosophy

* Assess and evaluate patient needs, establishing a personalized Plan of Care

* Perform initial OASIS assessments and ongoing re-evaluations

* Ensure patient safety through thoughtful care planning and resource coordination

* Collaborate with physicians, supervisors, and interdisciplinary team members

* Implement and revise Plans of Care based on patient progress

* Provide direct speech-language pathology services according to clinical standards

* Select therapeutic materials tailored to patient needs

* Educate and support patients and caregivers on home therapy techniques

* Maintain accurate, timely documentation and communication

* Work in a dynamic, physically demanding environment with flexibility for irregular hours

What You'll Need

* Master's degree from an accredited program in Speech-Language Pathology

* Current, unrestricted SLP license in Indiana

* Minimum 1 year of clinical SLP experience

* Ability to travel 50% within your service area

* Reliable vehicle, valid driver's license, and auto insurance

* Ability to lift 50-100 lbs. and assist with patient transfers
