Job Description:
We are seeking an enthusiastic and compassionate Family Medicine Practitioner—either a Nurse Practitioner (NP) or Physician Assistant (PA)—to join our team. The ideal candidate is passionate about rural healthcare, efficient, resourceful, and dedicated to delivering the highest level of patient care.
- Provide comprehensive primary care services to patients in a rural setting.
- Educate patients and families about health conditions, treatment plans, and preventive care.
- Assist patients in navigating healthcare resources, ensuring they receive needed care either in‑house or through referrals.
- Provide in‑house lab testing and perform a variety of procedures to meet patient needs.
- Collaborate with the healthcare team to deliver holistic, patient‑centered care.
- Maintain accurate and detailed patient records, ensuring compliance with all regulations and standards.
- Contribute positively to our family‑oriented workplace culture through collaborative teamwork.

- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Indiana.
