Home Health Registered Nurse, Case Manager

• Perform all scheduled home health visits
• Collaborate with members of the interdisciplinary team
• Conduct physical, psychosocial and environmental assessments of assigned patients
• Update plans of care based on visit findings
• Follow care plans, evaluate outcomes and apply appropriate interventions
• Communicate with physicians to confirm and update patient treatment plans
• Communicate treatment-plan changes to the RN Case Manager
• Assess patient responses to treatments, medications and teaching
• Revise care plans as indicated
• Partner with the RN Case Manager on discharge planning
• Utilize interagency, system and community resources to ensure continuity of care
• Accurately document and update patient records according to agency, state and federal guidelines
• Communicate problems, plans, actions and goals to physicians and the RN Case Manager
• Follow and monitor published policies and procedures
• Perform patient positioning and transporting duties using proper lifting and pushing/pulling techniques
• Provide age-appropriate care based on patient status and developmental needs

• Registered Nurse license issued by the state in which the team member practices
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) upon hire unless determined not required
• Valid driver's license
• Completion of an accredited or approved program in Nursing
• Typically 1 year of experience in nursing
• Proficiency in clinical skills with ability to work under direction and make sound judgments
• Excellent communication and interpersonal skills
• Ability to develop rapport and maintain positive, professional relationships with patients, staff and physicians
• Ability to make critical, independent decisions
• Organization, prioritization and problem-solving skills
• Ability to multitask with frequent interruptions
• Ability to sit, stand, walk, lift, squat, bend, reach above shoulders and twist frequently
• Ability to lift up to 50 lbs. from floor to waist
• Ability to lift up to 20 lbs. over the head
• Ability to carry up to 40 lbs. a reasonable distance
• Ability to push/pull with 30 lbs. of force
• Ability to perform a sliding transfer of 150 lbs. with a second person present
• Ability to use protective clothing and equipment when necessary
• Functional speech, vision, hearing and touch, with fine hand manipulation skills
• Ability to operate all equipment necessary to perform the job

Demonstrates proficiency in nursing practices, including conducting assessments, updating care plans, and collaborating with interdisciplinary teams. Possesses strong communication skills to effectively interact with patients, physicians, and case managers while ensuring compliance with healthcare guidelines.
