As an Evening Shift Licensed Practical Nurse (LPN) at Greendale Park Nursing & Rehab, you will play a vital role in delivering high-quality patient care, working closely with our interdisciplinary healthcare team to ensure the well-being and satisfaction of our residents. Your evening shift will span [insert hours] and will involve providing direct patient care, administering medications, and documenting patient information.

• Provide skilled nursing care to residents, including administration of medications, wound care, and vital sign monitoring
• Develop and implement personalized care plans, collaborating with other healthcare professionals to ensure comprehensive care
• Maintain accurate and detailed records of patient care, medication administration, and resident progress
• Monitor and report changes in resident condition to the charge nurse or other healthcare professionals
• Participate in the development and implementation of individualized care plans, working closely with residents, families, and healthcare team members
• Perform other duties as assigned by the charge nurse or other healthcare professionals

• Must be a licensed practical nurse (LPN) with current certification and registration
• High school diploma or equivalent required; associate's degree or post-graduate certification in nursing a plus
• Strong communication, teamwork, and problem-solving skills
• Ability to adapt to a fast-paced environment and prioritize multiple tasks simultaneously
• Must be comfortable working in a long-term care setting and with individuals with diverse needs and abilities
