IRIS Program Consultant

Lutheran Social Services seeks an IRIS Program Consultant to support participants in self-directed, community-based living. In this role, you will provide person-centered consultation, assess needs, and develop individualized IRIS service plans that promote independence, dignity, and choice. You will educate participants and families on program options, coordinate services with providers, and advocate for access to housing, behavioral health, and supports.
