Care Coordinator

Role Overview:

This position involves serving as a primary contact for families of children with developmental, physical, or severe emotional disabilities, supporting their access to community-based services through assessment, planning, coordination, and ongoing support.

• Conduct functional assessments and develop individualized service plans with families and providers
• Authorize, coordinate, and monitor services in alignment with plans
• Facilitate family-centered team meetings and maintain accurate documentation
• Collaborate with service teams and supervisors to determine interventions
• Testify at legal proceedings when necessary
• Participate in staff development, supervision, and training

• Promote client independence and growth through trauma-informed, family-centered services
• Utilize evidence-based practices supported by organizational and funding guidelines
• Accurately complete time reporting

• Bachelor’s degree in a health or human services field
• At least 1 year of experience working with children in the target population
• Strong communication, documentation, and organizational skills
• Proficiency with electronic health records and computer applications
• Valid driver’s license and reliable transportation
• Bilingual fluency in Spanish is preferred but not required
