* Purpose:

To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.

Qualifications:

* Must successfully complete the required pre-employment Background and Drug Screenings
* Must possess a valid Driver's License which meets company standards for approved driver status
* Must possess advanced computer skills
* Must possess a High School Diploma or equivalent, Bachelor’s Degree preferred
* Experience working in the field of intellectual/developmental disabilities preferred
* Minimum one year of experience in writing program plans, case notes or other relevant documentation required

Essential Functions:

* Responsible for the development, implementation and coordination of individual’s program plans
* Develop, implement and monitor individualized training programs to meet identified priority training needs
* Ensures all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possible
* Ensure all documentation is accurate and complete according to regulatory requirements
* Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed
* Performs other duties as assigned
