Responsibilities- Review resident information and evaluate its impact on assessment and treatment.
- Conduct and document thorough functional assessments using standardized procedures.
- Develop and implement individualized treatment plans with achievable goals.
- Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.
- Instruct and supervise therapy assistants as needed.
- Enter treatment and progress notes for each session.
- Revise treatment plans as needed throughout the course of care.
- Document treatment in clear, concise discharge summaries.
- Orient residents to services and treatment procedures appropriate to their needs.
- Collaborate with nursing staff, residents, and families to maximize functional abilities.
- Educate residents and caregivers about continuing home exercise and therapy.
- Assist with referrals for additional services as needed and maintain knowledge of community resources.
- Participate in rehabilitation team conferences and other facility meetings.
- Access continuing education opportunities to maintain clinical competency.
- Participate in quality improvement initiatives.
- Follow regulatory, professional, and facility standards.
- Perform other duties as assigned to provide quality care.
- Understand payer reimbursement and allocate therapy resources efficiently.
- Support Ahava’s mission and project a professional image.
- Maintain expected productivity levels.
- Bachelor’s or Master’s degree in Physical Therapy
- Current PT license in the state of practice
- BLS/CPR certification (current or obtained during orientation)
- Strong communication, teamwork, and organizational skills
